Athletic Forms

Final Forms must be completed and uploaded to the Final Forms site prior to tryouts, no exceptions.

  • Physical (uploaded)
  • Copy of insurance (uploaded)
  • Consent form (requires parent electronic signature)
  • Concussion form (requires parent and student electronic signatures)
  • Cardiac Arrest form (requires parent and student electronic signatures)

•Physicals must be dated after April 15, 2026 and are good through 2026-2027 school year. These will expire August 1, 2027.

• Sports physicals dated after April 15, 2025 are good until August 1, 2026. These physicals are good for summer tryouts and practices only.

**The Concussion-Return to Play form is only needed if a student is returning after a concussion: Concussion Form - Return to Play Form

Homeschool Athletics

Homeschool athletes must be cleared before participation. Contact Kim Owen with questions.

Gladeville Middle School 2026 Summer Tryouts

General Requirements and Athlete Information

  • Sign-up Requirement: All athletes must be signed up using the specific Google Form for each tryout to be admitted.

  • Paperwork: All required forms must be signed and/or uploaded to Final Forms.

  • Physicals: 2026-2027 seasons require a physical dated April 15, 2026, or later.

  • Physical Grace Period: Sports physicals from 2025-2026 are valid for summer tryouts only but must be updated before the school year begins.

  • TSSAA Dead Period: June 22 – July 5. All facilities are closed during this time.

  • Athletic Administration: Jordan Simpkins (Athletic Administrator) and Rachel Blagg (Athletic Director).

 

 

Sports
